I authorize my/my child’s participation at Safe Haven Ballet. I absolve Safe Haven Ballet staff or volunteers from any responsibility for accidents or injuries that may arise during class, while on the premises, or at a production. Furthermore, I recognize that a competent adult will be present at all events, practices, and productions. In the event of an injury or illness requiring additional treatment, I consent to my child being transported to the nearest hospital. I understand that any costs related to transportation, treatment, and other expenses will be the sole responsibility of the parent or guardian.
By registering myself/child(ren) with Safe Haven Ballet, I consent to my/their participation in Safe Haven Ballet activities. I hereby release Safe Haven Ballet, its directors, staff, board members, and volunteers from any liability for injuries that may occur to myself/child(ren), or family members while participating in Safe Haven Ballet programs, including travel to and from classes and productions. I understand that participation in Safe Haven Ballet is voluntary and recreational in nature. I agree to indemnify and hold harmless the organizations, individuals, agents, and employees against all liability for personal injury or property damage suffered by myself, my child(ren), or other family members during our involvement in any Safe Haven Ballet program.
I hereby consent and agree that Safe Haven Ballet and its agents have the right to take photographs, videotape, or digitally record dancers, and to utilize these materials in all media formats, both now and in the future. These materials may exclusively be used for the purposes of marketing, website content, and other promotional materials associated with Safe Haven Ballet.
